Neuropsychological Evaluation
What to bring: Glasses or contacts, hearing aids, beverages/snacks & medications if needed during the evaluation.
About the appointment: This neuropsychological evaluation assesses many aspects of cognitive abilities, including but not limited to, verbal and visual-spatial abilities, intellectual functioning, memory abilities, motor, effort, emotional, attention/concentration and reasoning. The session will last approximately 6-hours.
Our policy does not allow observation of actual test administration by third parties, such as family members, attorneys or their representatives, since such a presence would jeopardize test validity. Visitors are welcome to wait in the waiting area.
Please turn off all pagers/cell-phones during the evaluation to prevent distractions that will interfere with obtaining your best performance and valid results.
Directions From I-5:
Southbound: Take the James Street exit (165-A), turn left (East, up the hill), turn left onto Broadway, cross over Madison and make an immediate left. The office is located in a red brick building on the northwest corner of Broadway and Madison.
Northbound: Take the Madison Street exit, turn right onto Madison (up hill), turn left onto Broadway and make an immediate left again. The office is located in a red brick building on the northwest corner of Broadway and Madison.
Parking: A
pay parking lot is located on the northwest side of the building.
